This is mainly for Middle School students and Tdaps for the upcoming 7th graders but if anyone else needs immunizations they will provide them. They will be located in front of the Middle School at Oologah Public Schools on May 12th at 10:00am. There is a link on the website under the Parents and Immunization Information Section for a copy of IMMUNIZATION AUTHORIZATION Form.
