Oologah Middle School

The Caring Van will be here again this year on October 11th for providing free flu vaccines. This is for all staff and students. Students must bring back the consent form in order to receive the vaccine. The consent forms are below and can be printed off. The LE needs to have parents present for their kiddos. The van will be located outside the Middle School in the loop and will be here from 9:30-12:30. The consent form can be downloaded by clicking this LINK
